But what does “investigator” actually mean?
In healthcare SIU work, that distinction matters.
Many organizations use the term broadly — sometimes referring to analysts, researchers, or desk-based investigators who work primarily through databases, phone calls, records, and online research.
Those roles are valuable.
But they are not the same as having experienced field investigators who are actually part of the organization’s investigative team.
That raises an important question:
Are those investigators trained, licensed, integrated with analysts and medical reviewers, and consistently working cases?
That distinction can matter more than it first appears.
A dedicated employee investigative team builds familiarity with the organization’s processes, works directly alongside desk investigators and medical reviewers, maintains ongoing training and licensing, and develops a more connected approach to complex cases.
And when a case moves beyond what can be established from a database, the difference becomes even clearer.
A desk investigator may help determine the WHAT:
→ What address is listed
→ What license or credential appears in the record
→ What claims or documents were submitted
→ What relationships appear through link analysis
A field investigator helps determine the WHO — and whether the real world matches the record:
→ Who is actually at the address
→ Who is actually performing the billed services
→ Whether the patient is actually receiving the treatment
→ Whether a location, provider, witness, or relationship is what it appears to be on paper
Because data can show you what is documented.
Investigation can show you what is actually happening.
A mature SIU model is not just a collection of names on a staffing chart.
It is a connected investigative capability.
Desk intelligence.
Field validation.
Medical expertise.
Case development.
Actionable findings.
All working together.
Because when the stakes are high, “we have investigators” should mean more than someone who knows how to search a database.
